CAMP FORMS

The following forms are available.  All registration is done online.  Once you register you will receive both a confirmation email and an email with the remainder of forms which need to be completed prior to your camp arrival.  For records purposes, please return all forms to our office prior to the start of camp.

Airline Form
If your child will be traveling to camp alone please complete this form with the pertinent travel information.  We will need this as soon as possible so that we may make the necessary shuttle arrangements as well as staff accompaniment.

 Physician's Medical Clearance Form
Your child will  need a current physical or this form completed in order for them to attend camp.  Please be certain we receive this prior to camp check-in.

 

Swim Camp Rules
Just what it says...if you have not already recieved a copy of this form we ask that you print this out and review it in it's entirety.  When you have discussed all rules with your camper, please have them sign the form and return it us for their files.

What to Bring
Not sure what your child will need to bring to camp?  This form will provide you with some suggestions...Sunscreen and a rain poncho would also be GREAT additions!!!
